Your answers never leave your device

The short version When you take a test, your answers are scored entirely in your browser. They are not sent to our servers, not stored, and not linked to you. Close the tab and they're gone.

Test answers

Every screening on HealthSurvey runs on your device. The questions, the scoring, and your result are computed in your browser with JavaScript — no answer is transmitted to us. We could not see your responses even if we wanted to, because they never reach us. There is no account to create and nothing to log in to.

What we do collect

Like most websites, our host may record basic, non-identifying request logs (such as a truncated IP and browser type) for security and to keep the site running. If we add privacy-respecting, aggregate analytics, we will use a tool that does not track individuals across sites and will name it here. We do not sell data, and we do not run behavioral advertising on clinical pages.

Your rights

Because we don't collect your test answers or build a profile of you, there is no personal health record for us to share, sell, or lose. For any data our host does process, you can contact us with questions or requests consistent with GDPR and CCPA/CPRA.

Children

HealthSurvey is intended for adults. Our screening tools are not designed for use by children, and we do not knowingly collect data from anyone under 13.
